FORMAL LETTER: NEW CREDIT CARD

You found your credit card irreversibly damaged. Write a letter to your bank manager. In your letter, you should say:
Who you are?
What happened to the card?
What would you like him to do?
Write at least 150 words.
Dear Madam,
I am a current account holder in your bank and I am writing this letter to seek some information about my damaged credit card, which I found I can hardly use anymore.
It was during my last holiday to Switzerland; I met with an accident, and at the hospital where I was taken to, my wife found my credit card in a futile condition. It was on the last day of my holiday, when I went for trekking; I slipped my right leg from the sloppy hills, and fell down to the bushes. Immediately, my wife and my co- travellers took me to the hospital, where the doctor advised some further diagnostic tests. It was at this point of time, my wife searched for my wallet and found my credit card damaged.
I would like you to issue a new credit card as it aids my business activities. I would also like to know the requirements for the issuance. As you provide me the gold class service, I believe you would not insist me to come to you in person.
