 An explanation of what had happened and any possible evidence you may have
 Possible resolutions to consider.

MODEL ANSWER:

Dear Sir,

I am writing this letter to seek apology for failing to provide evidence for my absence of a semester exam, which was scheduled by our university.

Named as Lisa Bonnet, possessing the identity number: 1046 and case number JP4567 in the records of the University of Albion, I pursue Geography as my major, and this is just my first semester. As per the notification, my first semester exam for the main subject was scheduled on 7 June, and I was quite confident that I will successfully complete the semester without any backlogs.

Unfortunately on 6 June, I met with an accident which broke my right wrist and right leg, due to which I was hospitalized in the city hospital. I could not intimate you the incidence, and failed to provide any evidence, for which I seek apology.

I have my discharge summary with me, with which I can obtain a medical certificate within a day and produce before you soon.
